It unfolds in four steps: data collection to grasp the event, causal factor charting to map out contributing elements, root cause identification using decision diagrams to uncover fundamental issues, and generating actionable recommendations for prevention. RCA targets specific, manageable issues, ensuring durable solutions and future mishap prevention.
